The majority of my patients experience unexplained illnesses or are women with hormone problems. Prior to coming to see me, if you have significant or persistent symptoms, please consult your own General Practitioner who knows you well, to make sure that no serious or life-threatening disease is lurking in the shadows.
A typical situation for most patients coming to consult me:
Most people who consult me will have been carefully checked out symptom wise and reassured that nothing ‘serious’ is wrong with their health. Then, the issue for the patient is that, despite this reassurance, they will continue to experience symptoms of e.g. Severe aches and pains, persistent unremitting fatigue, woeful headaches, irritable bowel syndrome, recurrent infections, debilitating mood problems, lack of libido, insomnia, menstrual cycle related mood problems or whatever.
The patient, quite reasonably I think, will think: “Surely there must be something wrong which hopefully can be diagnosed and fixed”. This is the point at which I entered most patient’s lives.
A practical word of advice prior to coming to see me:
If you are coming to see me, we need to be able to work together over a minimum period of 4 to 6 months which will involve, as a minimum 4 - 5 consultations and may involve other treatments such as intravenous infusions etc. Consultations are different to the standard medical consultation in that they involve very comprehensive medical histories and examination of symptoms and sometimes, if we are not progressing well, a lot of brainstorming and re-examination of symptoms. It is a very time-consuming process and my consultations are significantly longer than in a traditional disease-based medical practice. If your funds are very tight or you're looking for a magical, instant cure, then I suggest save your money because I'm not a magician and to provide the best potential outcome for you the patient I need to follow an orderly working plan.
Please complete this questionnaire at least two working days prior to your consultation.
Please read the notes and examples carefully whilst you are completing the questionnaire.
Please type all your information in the boxes.
Please try to limit your answer to 200 words maximum per question.
The aim of the questionnaire is to provide a framework for the consultation. The first consultation will take 45 to 60 min. For a patient with a complex, unexplained illness, 45 to 60 min to gather full details even with a previously and carefully completed questionnaire may sometimes seem like a race against the clock.
The questionnaire has different sections each seeking information about different aspects of your health history and the aim is to form a 360° view of your health problems.
Naturally, this Questionnaire is confidential.
Please feel free to disregard any questions you are uncomfortable with.
